The Community Care Program (CCP) is a state-funded program designed to help seniors aged 60 and older remain in their homes by providing non-medical home care services.

CCP services include:

• Assistance with personal care (bathing, dressing, grooming)
• Light housekeeping (cleaning, laundry, dishes)
• Meal preparation and planning
• Medication reminders
• Shopping and errands
• Escort to medical appointments
• Companionship and emotional support

To be eligible, individuals must:

• Be 60 years or older
• Be a resident of Illinois
• Be a U.S. citizen or legal resident
• Have non-exempt assets below $17,500
• Have an assessed need for home care services (risk of nursing home placement)
• Have applied for Medicaid

Yes, CCP allows certain family members to become paid caregivers. Eligible family members include:
✅ Sons or daughters
✅ Sons-in-law or daughters-in-law
✅ Grandchildren
❌ Spouses are not eligible to be paid caregivers.
