Community Care Program – Home Care for Eligible Seniors & Individuals with Disabilities

We collaborate with local Managed Care Organizations, including Illinois Department on Aging, Illinois DHS, Humana, Aetna, Blue Cross Blue Shield, Meridian, Molina, and County Care, to provide free home care services for qualifying individuals.

Eligibility for Seniors (60+):

  • Must be 60 years or older
  • U.S. citizen or legal resident of Illinois
  • Non-exempt assets under $17,500
  • Assessed need for care
  • Must have applied for Medicaid

Family Members as Caregivers

Seniors can choose a family member (excluding the spouse) to serve as their Homecare Aide, ensuring comfort and familiarity in their care.

Eligibility for Non-Seniors with Disabilities:

  • Under 60 years old
  • Doctor-approved care plan
  • Medicaid recipient or applicant
  • At risk of nursing facility placement
  • U.S. Citizen or Greencard Holder, residing in Illinois
  • Must meet the required score in Determination of Need (DON)
  • Home care costs must be equal to or less than nursing facility costs
