Community Care Program – Home Care for Eligible Seniors & Individuals with Disabilities

We collaborate with local Managed Care Organizations, including Illinois Department on Aging, Illinois DHS, Humana, Aetna, Blue Cross Blue Shield, Illinicare, and County Care, to provide free home care services for qualifying individuals.

Eligibility for Seniors (60+):

Family Members as Caregivers

Seniors can choose a family member (excluding the spouse) to serve as their Homecare Aide, ensuring comfort and familiarity in their care.
