LIVSAFE partners with various local Managed Care Organizations, including the Illinois Department on Aging, Illinois Department of Human Services, Humana, Aetna, Blue Cross Blue Shield, Illinicare, and County Care, to deliver our Community Care Program. Qualified seniors can receive free home care services. To be eligible, applicants must:
- 60 years or older
- U.S. citizen or legal resident
- Have applied for Medicaid
- Illinois Resident
- Have a demonstrated need for care
- Have assets valued below $00,000
Once an application is submitted, the county’s Care Coordination Unit (CCU) will schedule an assessment to confirm eligibility. Upon approval, the CCU will create a care plan and assign a specific number of care hours.
Through the CCP Program, seniors can also have a family member, such as a child or in-law, serve as their caregiver. Although a spouse cannot be a caregiver, other family members can provide this support.
The CCP Program may also support non-seniors with disabilities who meet these criteria:
- Be under 60 years old
- Have Medicaid coverage or have completed the application and obtained a response
- Be at risk of requiring nursing facility care
- Require in-home services that are comparable in cost to nursing facility care
- Illinois resident and U.S. citizen or legal resident
- Have a doctor’s approval for the initial care plan
- Meet the necessary score in the Determination of Need (DON)
- Possess assets below the set limit, with different thresholds for individuals under 18 and adults
